Other signs include significant weakness preventing certain movements entirely, inability to raise or rotate the arm, and night pain disrupting sleep.

Diagnosis makes the difference

These symptoms should all signal a visit to a doctor or orthopedic surgeon who will assess the shoulder. Physical examination provides clues as doctors test specific movements, stressing different rotator cuff tendons. A physical exam alone, however, cannot definitively distinguish strains from tears. Magnetic resonance imaging (MRI) scans reveal the actual tissue damage. Ultrasound can also identify tears when performed by experienced technicians. X-rays do not show soft tissue damage but rule out other problems like arthritis, fractures, or bone spurs. For persistent symptoms beyond 6-8 weeks or significant weakness, MRI confirms the diagnosis and guides treatment decisions.
